Laser-Assisted Liposuction (SmartLipo)

Ultrasound-Assisted Liposuction

Ultrasonic-assisted liposuction uses high frequency sound waves emitted from the end of a specialty cannula to help rupture (or emulsify) fat and make its subsequent removal much easier. This gentle form of energy is the same as that used to visualize babies in utero and provides a safe yet effective means of liposculpture. One special form of ultrasound liposuction is VASER® Lipo or LipoSelection®, which uses tissue-selective ultrasound energy to target unwanted fat, preserving other important surrounding tissues blood vessels, nerves, and connecting tissue and promoting smoother contours with rapid healing.

Power-Assisted Liposuction

This term refers to a refinement of the liposuction procedure where the tip of the cannula mechanically vibrates (at speeds up to 5000 times per second) in order to break up fat cells and make their removal via suction easier. Power-assisted liposuction (or PAL) reduces the force required by the surgeon to manipulate the cannula, and can reduce overall procedure time and recovery.

Ideal Candidates for Body Sculpting / Liposculpture

The ideal candidate for surgical body sculpting or liposculpture is mildly to moderately overweight and in generally good health. They have a few problem areas that have not responded to diet and exercise and have realistic expectations of what a body sculpting or liposculpture procedure can do for them. If they have loose, flabby skin or a distended body cavity, a body lift procedure may be more appropriate (alone or in combination with liposuction) to achieve their desired profile. Traditional Liposuction vs Liposuction in 2016

The trend for many years now is to remove stubborn body fat with less and less invasive procedures. Traditional liposuction literally rips the fat out piece by piece with simple suction through a hollow metal cannula. Traditional liposuction usually requires general anesthesia, causes significant bleeding and postoperative bruising, traumatizes the skin resulting in sagging and poses higher risks due to the violent nature of the motion during general anesthesia. New liposuction techniques such as SmartLipo Ultra does not require general anesthesia as the actual motion is smoother and far less forceful . This technique uses heat from a laser and ultrasound source to melt the fat cells and help tighten the skin via warming of the collagen layer. Furthermore, since heat is the main agent, very little bleeding and subsequent bruising is seen. This aspect significantly shortens the recovery time to the point where patients are working in 2-3 days and lightly exercising in 5-7 days. The same amount of body fat can be taken out with these newer techniques but the ultimate factor is the surgeon wielding these amazing tools.

Difference between Coolsculpting and Liposuction

Liposuction techniques such as traditional liposuction and SmartLipo Ultra physically remove the fat cells via tiny incisions. The surgeon is still “going in” but newer techniques such as SmartLipo Ultra are far less invasive as compared to traditional liposuction. This newer, more effective technique utilizes laser and ultrasound heat to literally melt the fat cells before the surgeon gently suctions out the destroyed fat. No general anesthesia is used as only local anesthesia and oral sedatives are used. The heat from these powerful instruments also heat up the collagen skin layer, causing a tightening effect on the dermis. Since very little bleeding and bruising is seen with SmartLipo Ultra, the postoperative recovery period is significantly shortened. Most patients are mobile immediately and are back to work withing 2-3 days.
Coolsculpting belongs to the category of machines that try to destroy fat cells from the outside without going into the fat layer itself. All the damage is done from the surface of the skin. No incisions or general anesthesia is required. Coolsculpting “freezes” the fat cells to theoretically cause them to rupture and die. Multiple sessions are absolutely needed (anywhere from 4 to 8 sessions) and the results are mild and extremely variable. Initial claims by the Coolsculptingcompany were misleading because practitioners did not stress the necessity of multiple treatments. Too many providers were promising significant results after only one treatment. Also, the initial clinical studies claimed a reduction of 5-7 inches after Coolsculpting but they did not stress that this was cumulative amount from a total of ten body areas. It is still a commonly-held belief by many expert physicians that “you still have to go in and get the fat out.” Up to this point in time, no purely non-invasive fat reduction machine (Coolsculpting, Zerona, Thermage, ultrasonic cavitation, etc.) can match the efficacy of the minimally-invasive (SmartLipo Ultra) or more invasive (traditional liposuction) techniques.

Pricing of Liposuction/Liposculpting

As with most other practices, we price our SmartLipo Ultra treatment based on the number of areas targeted and the amount of fat initially present. Common areas include the abdomen, the flanks (love handles), upper back (bra fat), outer thighs, inner thighs, arms, neck and chest (men). We do not split the areas into right/left or upper/lower. For example, love handles are considered one area, not two separate areas. Generally speaking, one area will cost between $2500-2800, two areas will cost between $3600-4200 and three areas will cost $5600-6000. The cost includes the surgeon’s fee, the anesthesia, the staff and the surgical supplies. The compression garment is not included in the price as many patients prefer to purchase their own style of garment from a specialty store. Furthermore, if a patient is significantly obese, the prices may be higher to account for the extra time, effort and supplies to remove the fat. The most accurate way to learn about pricing is for the individual to come in to our office for a free evaluation by Dr. Chang.

How to Determine if You Are a Candidate for Liposculpting

In his busy practice, Dr. Chang feels that almost everyone is a candidate for SmartLipo Ultra as we all have either stubborn areas of fat or have generalized obesity. Whether the patient is extremely fit and has small stubborn areas that will not go away with exercise or the patient is significantly obese, most patients are candidates for SmartLipo Ultra as long as realistic and proper expectations are clearly outlined. Having performed over 2000 SmartLipo Ultra procedures, Dr. Chang is well aware of the goals and limitations of this procedure. He will clearly explain what is expected in terms of the fat reduction, change of body shape, recovery period, and skin expectations. Dr. Chang’s ultimate goal is to melt away as much fat as possible and to fundamentally change the patient’s shape for the better. Furthermore, Dr. Chang will honestly inform the patient if he or she is not a candidate for SmartLipo Ultra due to medical reasons, body fat distribution, minimal improvement in an area or improper expectations. you’ve always desired.
